Cross Over’s wheelchair evaluation, focusing on functional mobility, is a thorough assessment designed to identify the optimal seating system for patients. Our dedicated occupational therapists, specially trained for this purpose, collaborate with you to attain the highest level of mobility. Our Process

The process of getting a custom power wheelchair can take 30-60 days. Usually, your ALS clinic team will let you know when they think you should get started. 

You can initiate the process by talking to your physical therapist (PT), or occupational therapist (OT). 


Your PT or OT will recommend the right chair for you and submit the proper documentation so that you qualify with insurance and pay as little as possible out of pocket. In order to do that, you will need to follow these steps: 

Prescription

To begin, your physician must prescribe the custom power wheelchair in person and—for Medicare eligibility purposes—certify that you need it in order to complete basic activities of daily living (ADLs) inside your home. 

Assessment

Your physician will then order a power wheelchair evaluation, which will be conducted by a PT or OT along with an assistive technology professional (ATP) from a durable medical equipment (DME) company.

Submit results

If you qualify, your PT or OT will submit your evaluation results and medical documentation to Medicare, Medicaid, and private insurance for authorization.

Discuss details with insurance

After communicating with your insurance provider(s), the DME company will contact you to discuss what your out-of-pocket costs may be. 

Delivery

When the DME company receives the wheelchair from the manufacturer, they will contact you to schedule delivery to your home. 

Training and adjustment

On the same day or shortly after the equipment is delivered, the ATP from the DME company and/or your PT or OT will visit your home to spend an hour or two fitting your chair and training you how to use it.

This makes a professional wheelchair assessment vital for anyone using a mobility device, either powered or a custom manual wheelchair.


A comprehensive wheelchair evaluation will assess personal needs for mobility, custom seating, posture, and positioning. Multiple manual and power wheelchair models are available and recommendations for equipment will be made to optimize function, independence, skin integrity and postural alignment. A licensed therapist and qualified Assistive Technology Professional (ATP) will guide you in making personalized decisions to address your mobility needs and goals.

Postural Assessment

Posture Assessment involves collecting measurements to fit the wheelchair seating system to the individual by addressing sitting balance and postural deviations to minimize potential for pressure sores.

Pressure Mapping

Pressure mapping is a mapping system that identifies the areas of highest risk for skin breakdown to determine the most appropriate wheelchair cushion and backrest to effectively distribute pressure. 

Training

Education & Training is an integral part of all services provided. From wheelchair propulsion to weight shift techniques to positioning in the seating system, the therapists strive to make sure clients and caregivers fully understand and can safely operate their equipment.

Home Modification

Cross Over Mobility & Freedom strives to maximize your health, well being and quality of life to assist you in aging in place in your home or anywhere you call home. We can provide a comprehensive home safety and accessibility assessment by a licensed occupational or physical therapist. Our in-home evaluation includes a 60-90 minute interview and viewing of the home. The specialist and/or therapist will make the appropriate recommendations, review them with the individual and provide a comprehensive report. Cross Over Mobility & Freedom does not complete the actual modifications, but can refer you to licensed contractors, if needed.

Cross Over Mobility & Freedom strives to maximize your health, well being and quality of life to assist you in aging in place in your home or anywhere you call home. We can provide a comprehensive home safety and accessibility assessment by a licensed occupational or physical therapist. You will receive recommendations specific to your needs that may include home modifications, adaptive equipment to ease activities of daily living, referral for physical therapy evaluation, and/or fall prevention strategies.


Our in-home evaluation includes a 60-90 minute interview and viewing of the home. The specialist and/or therapist will make the appropriate recommendations, review them with the individual and provide a comprehensive report. Angel Leaf Rehab & Consulting does not complete the actual modifications, but can refer you to licensed contractors, if needed.
